Know Your Numbers 2014 Community Health Screenings

St. John’s Community Health Education Department is pleased to offer the following free and low-cost health screenings:

Blood Pressure – Free
Hemoglobin – Free
Body Mass Index – Free
Blood Sugar – Free
Hemoglobin A1C (HbA1C) – Free
Total Cholesterol, HDL, Total Cholesterol/HDL Ratio, Triglycerides – $10*

Screenings are available on the second Wednesday of every other month from 8:30 a.m. – 11:30 a.m. on the following dates:

• January 8
• March 12
• May 14
• July 9
• September 10
• November 12

Screenings will take place at St. John’s Regional Medical Center, located at 1600 North Rose Avenue in Oxnard in the Community Health Education Department (Garden Level).

St. John’s reserves the right to adjust fees, and cancel or reschedule any service. To receive a refund, at least 72 hours notice is required.

*Advance payment is required. Fasting for 12 hours is required for the test that includes triglycerides.
